My child has permission to attend the Bellevue Montessori School Main Campus for various events, nature walks to Hidden Valley Park,and  lunch outside the Rossano Campus. You will be notified in advance of upcoming events. 

By signing this statement, I release Bellevue Montessori School Rossano Campus, its teachers, staff, directors, and parent volunteers, and all other persons from responsibility for any liability which may occur while away from school.

I understand the school will provide notice in advance of specific dates, time and requirements for the above activities.  I am aware that my child is not required to accompany the school on a field trip or outing.  I understand that I can withdraw permission for participation at any time, but that it is my responsibility to communicate any change in writing to Bellevue Montessori School Administration.

I hereby agree that my consent for emergency care for my child as detailed in the Consent for Emergency Treatment Form will be in effect on field trips during school hours.
